Description

Cortisol tests are done to measure the level of the hormone cortisol in the blood. Cortisol is a corticosteroid hormone produced by the adrenal cortex (in the adrenal gland). It is a vital hormone that is often referred to as the "stress hormone" as it is involved in the response to stress. It increases blood pressure, blood sugar levels and has an immunosuppressive action. Cortisol levels increase when the pituitary gland releases another hormone called ACTH. A cortisol test may be done to detect problems with the adrenal glands or pituitary gland.

Cortisol has many functions. It helps the body break down food for energy (a process called metabolism), and it helps regulate the immune system, which helps the body manage stress and fight infection. Cortisol levels can be affected by many normal conditions, such as physical or emotional stress or strenuous activity. Abnormal conditions, such as infection or injury, can affect cortisol levels as well. A cortisol test may be requested if your doctor notices symptoms suspicious of Cushing's syndrome (high blood pressure, obesity, muscle wasting, and muscle weakness) or Addison's disease (low blood pressure, weakness, fatigue, increased pigment on the skin among others), and wishes to make a diagnosis.

Normally, cortisol levels rise during the early morning hours and are highest in midmorning (about 8 a.m.). They drop very low in the evening and during the early phase of sleep. However, if you sleep during the day and are up at night, this pattern may be reversed. The absence of this daily variation in cortisol levels may be one of the first signs of overactive adrenal glands, especially Cushing's syndrome.
